The Phoenixville High School has organized its transition support into high-incidence disabilities and low-incidence disabilities.

Students with high-incidence disabilities make up 80%of all students with disabilities, which often include the following IDEA categories:

Speech and Language Impairment

Learning Disabilities

Emotional Disturbance

Mild Intellectual Disabilities.

Students with low-incidence disabilities make up 20%of all students with disabilities, which often include the following IDEA categories:

Intellectual Disabilities

Autism Spectrum Disorder

Hearing Impairments

Orthopedic Impairments

Visual Impairments including Blindness

Deaf-Blindness

Deafness

Other Health Impairments

Traumatic Brain Injury

Multiple Disabilities

 


At the age of 14, IEP teams begin activities to prepare for a student's transition from secondary education toward postsecondary goals of education, employment, and independent living.  This would start no later than the annual IEP during which a student turns 14.

The Three Postsecondary Goal Areas

Postsecondary Education

Postsecondary Education is education and training, which occurs after high school graduation.  This may include a four-year college, community college, a vocational training school, or other forms of education.  Click here for additional information on this postsecondary goal area.

Postsecondary Employment

Postsecondary Employment may be competitive employment with or without support.

Postsecondary Independent Living

Postsecondary Independent Living addresses whether an individual can live on his/her own or if he/she will require support.
