The Health Room is located on the first floor next to the main office (A-4) and is staffed Monday-Friday from 7:10am – 2:40pm. The office is open to all students and staff of PAHS. All students must have a pass for admission to the nurse’s office. Exceptions are made in emergent situations such as bleeding, head injury, difficulty breathing, or vomiting.

Our goal as school nurses is to help students incorporate healthy behaviors and achieve optimal health status which enhances their ability to learn. Our nursing interventions encourage student responsibility and independence in becoming healthy adults.

Each student is required to have Emergency Information that includes medical authorization and consent electronically signed by a parent or legal guardian. Please complete the online form including any changes regarding address, telephone number, medical conditions or medication changes, and submit on the PASD Website under (annual Health Information Update) Skyward. This form is not carried over to the next year and must be completed on a yearly basis. 

All students leaving the building due to illness MUST be dismissed through the nurse’s office. When a student is sent home an Illness Dismissal and Readmittance Form is sent with them to be signed by a parent/guardian and returned to the Attendance Office. Students are to sign out with the Front Desk prior to leaving the building. Transportation is the responsibility of a parent/guardian.

When it is necessary for medication (excluding parental permission for standing order medications (Tylenol, Ibuprofen, Tums, Benadryl) to be given to a student during school hours, a medication consent form (PH-17/a) must be completed and returned to the school nurse by the parent/guardian; medication must be delivered to the school nurse in its original container (students are not permitted to carry medications with them in school) along with a doctor’s written prescription.
